Please use the following process for requesting your medical records.

Request for Medical Record Copies will be handled by Ciox.

It is required by law to obtain your permission before releasing any copies of your medical record. Copies of medical records that are sent directly to health care providers will be provided at no charge. If you wish to receive a copy of your medical records, please click here. To check on the status of a previously submitted request, please call 1.800.367.1500.

Request for Same Day Medical Record Copies

For same day retrieval of records or images, visit our Northwest office (only) to fill out the Request for Medical Records authorization.
